Bend & Banter Participant Waiver, Release & Media Consent

Voluntary participation. I am voluntarily taking part in a stretching, mobility, and movement session provided by Bend & Banter. I understand these sessions involve physical activity, including guided and assisted stretching.
Assumption of risk. I understand physical activity carries inherent risks, including muscle strain, soreness, injury, or aggravation of a pre-existing condition, and I voluntarily accept these risks. I confirm I am physically able to participate. I will inform the instructor of any injury, limitation, pregnancy, or medical condition before beginning, and will speak up immediately if any movement causes pain.
Not medical treatment. I understand Bend & Banter provides general movement and stretching for wellness and is not medical care, physical therapy, or treatment for any condition. No specific health outcome is promised. For medical concerns, I will consult a licensed professional.
Assisted stretching consent. I understand some stretches may involve the instructor's hands-on assistance. I consent to appropriate physical contact for this purpose and understand I may decline or stop any hands-on assistance at any time, for any reason.
Release of liability. To the fullest extent allowed by law, I release and hold harmless Bend & Banter and The Don Creations from any claims, liabilities, or expenses arising from my participation, except in cases of gross negligence or willful misconduct.

Media consent, sessions may be photographed, filmed, or audio-recorded for Bend & Banter's content (social media, YouTube, marketing).

Yes, I'm happy to appear in Bend & Banter's photos, videos, and audio. I give Bend & Banter (The Don Creations project) permission to share and use this content, including my appearance and voice, across social media, YouTube, and promotional material, now and going forward, at no cost to either of us. Bend & Banter agrees this footage will be used as genuine recordings and will not be altered or recreated using artificial intelligence.
No thanks, I'd prefer not to be photographed or recorded. Please keep me off camera.
